Health Check Scorecard

You can score points by completing Health Checks, like a comprehensive HTC visit. This encourages you to stay healthy while earning points for your team!

If you download your challenge or Health Check Scorecard, please write your unique ID in the space provided on the challenge or scorecard. Unique ID numbers are preprinted on all materials included in the player kit. If you lose your unique ID, contact HealthInfo at 1-866-383-8166 or info@healthinfodirect.com. Enclose the Challenge or Health Check Scorecard in one of the prepaid envelopes sent with the player kit. You must be registered by your chapter in order to submit downloaded challenges.

You are not required to have a signature from your healthcare provider to complete your Health Checks. Rookies, Juniors, and Seniors simply need a parent or caregiver's signature to confirm your completion. (Graduates can sign their own Health Checks using the honor system.)
